The following forms and important documents are provided for your assistance with establishing an Account in the Plan. Please make sure to download and review the Enrollment Handbook and Supplements along with any applicable prospectuses from the "Investment" section. Then simply download, print and complete the appropriate forms and mail to:

• Enrollment Form | Used for opening a College Savings Plan of Nebraska Account. |
• Enrollment Handbook | Important legal document explaining and providing details on the Plan. |
• Program Brochure | Information and highlights of the College Savings Plan of Nebraska. |
• Payroll Deduction Form | Use this Form to contribute to the Plan via payroll deduction. |
• Rollover Form | Use this Form to rollover funds from another 529 Plan or to move funds between College Savings Plan of Nebraska Accounts. |
